RISE and NADAP provide social services

Two different social services agencies have opened in Patchogue this year.

RISE Life Services of Riverhead opened a day program for adults with developmental disabilities, which it operates from space leased from St. Paul’s Episcopal Church.

And NADAP, a Manhattan-based nonprofit, held a grand opening on Nov. 16 for its new office at 180 W. Main Street.

NADAP provides case management services for Medicaid patients with chronic medical conditions, mental illness or substance abuse disorder under a contract with the New York State Department of Health.

The program aims to connect patients with organizations that provide medical care and makes sure clients have the necessary medications and are taking them and following up with doctor visits.

The idea is that if chronic medical conditions such as diabetes are managed, patients will be healthier and won’t have to turn to hospital emergency rooms for treatment, which is costly.

Clients often have other issues that impact their health, such as lack of healthy foods or housing. NADAP connects clients to service providers that can assist with those needs, said Gina Caporaso, NADAP’s director of health homes operations.

The program takes “a holistic view of the person making sure all of their needs are met,” Caporaso said.

Patchogue is NADAP’s second office on Long Island. An office in Hempstead prepares people with job readiness skills and helps them find employment.

About 10 people are enrolled in RISE Life Services’ Without Walls program, which helps adults with developmental disabilities become part of the community by participating in activities at organizations such as the YMCA, Patchogue-Medford Library, and helping out at the soup kitchen and thrift shop at St. Paul’s.

The program is looking to enroll additional people in Patchogue, said JoAnn Vitale, operations director of day and community services.

RISE Life Services also operates group homes for adults with developmental disabilities.
